(WBBM NEWSRADIO) -- Cook County Sheriff Tom Dart is encouraging parents whose children were patients of an Evergreen Park pediatrician to make sure their children were properly vaccinated.

Dart says Dr. Van Koinis was found dead from a self-inflicted gunshot wound in September in a forest preserve in Palos Township.

Koinis left a suicide note that raised questions about record-keeping of vaccinations at his office on 95th Street in Evergreen Park, the sheriff said Tuesday.

In addition to not being clear which children got vaccinations and which didn’t, Dart says it’s possible the doctor did not provide vaccinations to some children, despite requests from their parents.

Dart is urging parents whose children were patients of Koinis to check with their current doctors about ways to test for whether their children got their vaccinations.

The suicide remains under investigation and the state health department has been notified.
